SUPPORT AT HOME
The Support at Home program begins on 1 November. It will replace the Home Care Packages (HCP) Program and Short-Term Restorative Care (STRC) Programme. Support at Home is part of the new Aged Care Act, which puts the rights of older people first.
Under Support at Home, you will have better access to services, products, equipment and home modifications to help you remain healthy, active and socially connected.
Support at Home Program includes:
- 8 Ongoing Classifications
- 8 Ongoing Classifications based on your needs
- 3 Short-term Support Classifications:
>Assistive technology and home modifications
>Restorative Care Pathway (helping you regain independence after illness or hospital stay)
>End of Life Pathway (compassionate support for individuals and families during the final stages of life) Chinese Welfare Services' team are bi-lingual in Chinese/English and are qualified with a minimum of Certificate III in Individual Support (Ageing, Home and Community), plus receive regular staff development training and updates.

How to access SAH program:
- Call My Aged Care on 1800 200 422
- visit www.MyAgedCare.gov.au to register and to receive an assessment.
- If you need an interpreter, call the Translating and Interpreting Service 131450 then ask them to call My Aged Care. You can nominate CWS as your ‘preferred provider’ at the time of assessment.
